Application for Nursing Licensure Exam at PRC Lucena

Arrive early to avoid the long line.

Someone will collect your Original Cedula.

Fall in line based on the order of your Cedula. The PRC Guy will call your name one by one.

Be sure you have the documents in the following order:

• 3 pieces Passport size Photo with White Background and Name Tag
• Original NSO Birth Certificate --- will be returned to you
• Original NSO Marriage Certificate (for married women)
• Original Transcript of Records --- will be returned to you
• Photocopy of NSO Birth Certificate
• Photocopy of Transcript of Records
• Photocopy of NSO Marriage Certificate (for married women)
• Original RLE Summary
• Original 5x5 Cases

When you get to the application window, you will be given the

• Nurses' Application Form (NAF)
• Notice of Admission (NOA)
• A small PRC Card (reminds of the signature specimen card at the bank)

Accomplish them accordingly, and proceed to Cashier Window to pay Exam Fee of 900.00

Attach your 3 passport photo to your NOA, NAF, and "small PRC Card" accordingly.

Affix your right thumb mark on the "small PRC card" and the NAF

Proceed to the final window, and submit all the documents you've been processing so far.
Answer any questions the PRC Guy may have.
Pay the fee of 75 (forgot to ask what that was for, no receipt)
You will be given back:

• the processed NOA, 
• your Exam fee receipt, 
• a letter with instructions for exam day
